New Patient Forms
Since it will be your first time at The Dentist of Summerlin, we ask that you please complete the New Patient forms. Evaluating your medical history allows us to get a better understanding of your dental care needs. The forms will take some time to fill out, so we recommend coming 10-15 minutes before your appointment if you are unable to fill out the forms beforehand.
In our effort to reduce paper use and waste, we have a link to our online New Patient forms that takes the information you input and places it in your patient chart without the need for paper.
However, if you would rather print and fill out our New Patient forms you may use the following document and bring the completed forms with you to your appointment.
